Community Health Worker
Tri-Town Community Action Agency
Tri-Town Community Action Agency is actively recruiting for a Community Health Worker. The duties of the Community Health Worker include, but are not limited to, assisting patients of the Health Center to navigate systems ensuring completion of orders and referrals. The ideal candidate would have knowledge of the Johnston and North Providence areas, and hold a certification as a Community Health Worker, or be willing to become certified. The Community Health Worker also supports the mission of the Tri-Town Community Action Agency and the Health Center in the community by acting as a liaison between the Health Center and wider health professional community.
